Pre-Kindergarten - children must be 4 years old by August 1st.
Class Days: Monday through Thursday
Full Day Kindergarten - children must be 5 years old by August 1st
Before & After Care and Out of School Time- Kindergarten through Fifth Grade. Transportation to and from Jonathan Jennings and Pleasant Ridge Elementary Schools. Care provided during fall, winter and spring breaks, snow days and weather delays.
Summer Camp - for 3 year old to 12 years old
Child Care - children 3 - 5 years old
Before & After Care - Kg - 5th grade
Infant & Toddler Care - children 6 weeks to 3 years old

I, Rita C. Poff principal of St. Michael, do hereby certify under the penalties for perjury that following statements are true and correct:
1.
I have served as principal of St. Michel during the previous school year.
2.
That the school is owed an operated by St. Michael, Catholic Church, Inc., (“Church”). That the Church, as a part of it bylaws, has adopted a policy of racial nondiscrimination, which is attached hereto as Exhibit A., (“Nondiscrimination Policy”).
3.
The records of the school establish compliance with the Nondiscrimination Policy.
4.
All scholarship or other comparable benefits provided by the school are offered on a racially nondiscriminatory basis. The availability of such financial assistance is known throughout the general community being served by the school.
5.
The school includes a statement of its Nondiscriminatory Policy as to students in all its brochures, catalogues, advertisements and websites dealing with student recruitment, admissions, programs, and scholarships.
6.
The Archdiocese on behalf of all of its schools has published the following notice in the Criterion and other newspapers of general circulation:

All schools operated by schools and parishes under the guidance of the Roman Catholic Archdiocese of Indianapolis admit students of any race, color, national and ethnic origin to all the rights, privileges, programs, and activities generally accorded or made available to students at the school. The schools do not discriminate on the basis of race, color, national and ethnic origin in administration of its educational policies, admissions policies, scholarship and loan programs, and athletic and other school-administered programs.

I authorize the Office of Catholic Schools of the Roman Catholic Archdiocese of Indianapolis, Inc. to file Form 5578 on behalf of the school.
